Lead Physician, Family MedicineAbout RoleAs a Lead Provider at
Vera Whole Health, you will set the standard for clinical
excellence to keep all care centers running smoothly. You will
partner with the Regional Lead Provider and Regional Operations
Director to ensure the delivery of an exceptional patient
experience. Both a clinician and a leader, you demonstrate deep,
authentic care for your patients and care teams. You are focused on
building strong relationships with your patients to address their
needs beyond their symptoms. You meet with 10 to 15 patients each
day for 30 to 60 minute appointments, supported by an entire care
team, including health coach, to deliver whole person care - care
that's often outsourced to specialists in today's broken system.

(EAP)Essential FunctionsYou will:* Uphold the mission, vision, and
values of Vera Whole Health* Provide all care and duties required
of a provider* Lead the onsite primary care team to improve patient
health through driving participation in annual whole health
evaluations, health coaching, and other health promotion
activities* Support change management and comprehensive care
management for both individual patients and client populations*
Provide an optimal patient experience in your care center,
improving workflow where possible, ensuring prompt resolution of
patient complaints, and driving team member engagement* Partner
with the Regional Lead Provider, Regional Director of Operations,
and Practice Manager to ensure all customer needs and expectations
are met, which includes open communication, regular review of
contractual metrics, and development of clear action plans with
prompt implementation to address evolving customer needs* Work with
your care team to achieve organizational goals related to clinical
quality, patient satisfaction, patient engagement, and quality
outcomes metrics* Participate in organizational activities and
committees (e.g. weekly provider meetings, Clinical Advisory
Committee meetings, Quality Improvement Committee, etc.) to
optimize Vera's processes and programs* Participate in
miscellaneous outreach endeavors to drive patient engagement, such
as biometric screening events, employee health fairs,
lunch-and-learns, etc.* Rotate with other providers in the region
to ensure coverage after-hoursEducation & ExperienceYou have:* MD,
DO, NP or PA Board Certification with primary care, family
medicine, or internal medicine background* Current, valid and
unrestricted medical or ARNP license and valid DEA number* 3-5
years of primary care experience* 2+ years with case management
emphasizing health prevention and population health management* 1-2
years leading team-based care* Experience working in EMR systems,
NextGen preferred* Experience in coding and documentation,
including Medicare Advantage and HCC* Experience with MS Office
suite
