Registered Nurse - VA Community Care - Amarillo, Texas
Duties

The RN Care Coordinator/Case Manager functions directly to support
the activities of the local Office of Community Care (LOCC) and
support the mission of the Office of Integrated Veteran Care (OIVC)
through creating a seamless and coordinated healthcare experience
for the Veteran who accessed the VHA system, its facilities, or the
in the community. The Registered Nurse is responsible for the
planning, integrating, implementing, modifying, and evaluating the
effectiveness of the Veteran's care. The LOCC foundational elements
will serve to support delivery of high quality, timely care to the
Veteran receiving services in the community. The Registered Nurse
is the lead of the integrated care team established by the VA
Community Care Operating Model. He is responsible for providing
clinical leadership and expertise on community care processes and
regulations. The Registered Nurse will serve as a liaison to both
internal and external providers and will be responsible for
managing the care of the Veteran and the seamless transition to and
from the community. She possesses the knowledge related to VHA
guidelines, current evidence based standards of care, compliance
guidelines, and local policy related to Delegation of Authority as
established by the facility Chief of Staff.
The RN functional role includes following the standardized care
coordination model, incorporating case management principles if
applicable, knowledge of processes related to referral and
authorization and secure medical documentation exchange. The
ability to identify Veterans requiring disease management and the
process to obtain these resources internal or external to the VA if
applicable. The RN will focus on providing care that is unique to
the Veterans needs and continues to monitor, track, and intervene
if necessary in the episode of care across the continuum. The
Registered Nurse will demonstrate effective communication skills to
collaborate and co-manage the Veterans care with the
multidisciplinary team in a cost-effective manner.
The Community Care Coordinator RN must have multi-disciplinary and
clinical decision making experience in the acute care setting and
utilization review knowledge, including the ability to match a
clinical picture with interventions and evidenced based criteria to
manage multiple conditions. The RN role also requires familiarity
with CMS' National and Local Coverage Determinations, CMS general
compliance, coding, and billing as well as extensive knowledge in
the entire journey of care. The Veteran's journey of care includes,
but is not limited to inpatient, surgical, and ambulatory care,
home care, behavioral health care, recovery and chronic care, and
transitional care; which also includes care co-ordination and
guidance to bring the Veteran back from the clinic sent to in the
community.
Pay: Competitive salary, regular salary increases, potential for
performance awards
Paid Time Off: 50 days of paid time off per year (26 days of annual
leave, 13 days of sick leave, 11 paid Federal holidays per
year)

Licensure: 1 full and unrestricted license from any US State or
territory
