Job Description:

Encompass Health is seeking a compassionate and diligent Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our dedicated healthcare team. In this vital role, you will provide direct nursing care to patients in our diverse service areas, including home health, inpatient rehabilitation, and hospice care. Our ideal candidate will deliver high-quality care focused on enhancing patient recovery and quality of life in post-acute settings. This position requires a strong commitment to patient health, teamwork, and ongoing professional development. Administer nursing care to ill, injured, or disabled patients within home health, inpatient rehabilitation, and hospice settings. Monitor and report changes in patient symptoms or behavior to the supervising registered nurse and attending physicians. Maintain accurate patient medical records and operate medical equipment safely. Educate patients and their families on disease prevention and health maintenance measures. Coordinate with healthcare team to assist in patient recovery and enhance outcomes. Proficient in providing patient-focused care Strong organizational and communication skills Able to operate medical equipment Comprehensive knowledge of health and safety guidelines Medical Insurance Dental Insurance Vision Insurance 401k Current LPN or LVN license in the state of practice Basic Life Support (BLS) certification preferred
