Job Description:
Encompass Health is seeking a dedicated LPN / LVN to join our
compassionate team, dedicated to providing superior care in home
health, inpatient rehabilitation, and hospice settings. As part of
our team, you'll play a crucial role in assisting patients as they
overcome serious illnesses or injuries, ensuring their journey to
recovery is both efficient and comforting. This position requires a
combination of clinical excellence, compassionate care, and a
commitment to the highest professional standards. Administer
nursing care to ill, injured, or disabled patients within a home
health, rehabilitation, or hospice setting. Monitor and report
changes in patient symptoms or behavior. Communicate with
collaborating physicians or specialists regarding patient care.
Educate patients about health maintenance and disease prevention.
Facilitate referrals to other healthcare professionals and medical
facilities. Maintain accurate patient medical records. Provide
emotional support to patients and their families. Strong
interpersonal and communication skills. Ability to work well under
pressure. Solid understanding of health information structures and
technology. Experience with clinical tasks including IV insertion,
phlebotomy, and wound care. Medical Insurance Dental Insurance
Vision Insurance 401k Current LPN or LVN license valid in the state
of practice. CPR certification.
