LVN - ACO Care Coordinator - FT - Amarillo

Job Description:

The Licensed Vocational Nurse (LVN) is a non-exempt position responsible for performing a variety of duties depending on whether it is a small clinic, large medical practice, multi-clinic, or a specialty office. The LVN functions under the supervision of the Nurse Manager/Lead Nurse. The LVN assures provision of care to an assigned patient population, utilizing the principles of Performance Improvement in collaboration with other health team members. The LVN supports an organizational culture for Service Excellence and practices the Service Excellence Standards to all customer groups. Amarillo has a population of nearly 200,000 people. The public schools are outstanding and there are college preparatory and parochial private schools available. The city boasts many cultural activities, including the Amarillo Symphony and Opera, the Lone Star Ballet and a variety of theater groups. Recreational activities are plentiful. Amarillo offers a spectacular botanical garden, numerous art galleries and museums, and outdoor activities, including golf, hiking and biking. The city is located minutes from Palo Duro Canyon State Park, the second largest canyon in the country, and is on Historic Route 66. Northwest Texas Physician Network is recruiting one ACO LVN for the the Wyatt Clinic. The role of the ACO LVN is to maximize the components of care coordination to improve patient's healthcare experience and outcomes, by utilizing care coordination tools, criteria and protocols to provide patients with chronic and acute conditions that support, educate and assist in the prevention and/or maintenance of their disease and/or health and wellness state; increase patients compliance with treatment plans; engage community resources to support the patient's optimal functioning; and improve collaborative coordination of care to affect waste and inefficiency. implements and notes response to an age-appropriate plan of care utilizing assigned components of the Nursing Process, Clinical Skills, and Patient Education. The LVN delegates to unlicensed personnel in accordance with Nurse Practice Act. Also, the LVN provides an environment conducive to safety for patients, visitors, and employees. The LVN will report risks to patient safety and implement the appropriate Precautions. The LVN will comply with appropriate and approved standards.   • Graduate of an accredited school of nursing
  • LVN currently licensed by the State of Texas
  • Possess current BLS certification from the American Heart Association upon hire and maintain certification throughout employment
  • Minimum of one year of experience in clinic hospital setting, preferably primary care
  • At least one year of direct patient care experience utilizing an EMR
  • Bi-lingual preferred
  • Experience working with CMS preferred
  • Working knowledge of Microsoft Word, Excel, PowerPoint and Outlook at minimum intermediate level
  • Professional written and verbal communication skills
  • Effective time management skills
  • Ability to prioritize and schedule daily activities for the most efficient use of time
  • Interpersonal skills with the ability to perform with little supervision
  • Problem solving, analytical and interpretation skills
  • Strong decision making skills with the ability to investigate and weight alternatives and select the course of action that provides the greatest benefit to the patient and organization
  • Creative thinking skills with the ability to seek guidance when necessary and propose process improvements PHYSICAL AND SENSORY REQUIREMENTS
    • Ability to read and write in English
    • Ability to interact professionally and effectively with patients, families, staff, physicians, support agencies, vendors, and others
    • Ability to remain calm during stressful situations
    • Ability to respond to pages
    • Ability to comprehend and follow instructions
    • Bending, lifting (minimum of 25 pounds), grasping, fine hand coordination, pushing, pulling, and squatting
    • Ability to repeatedly twist, squat, and stoop
    • Exposure to infectious organisms, agents, and toxic chemicals
