Encompass Health is seeking a dedicated Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team in providing exceptional care to patients dealing with serious illnesses or injuries. In this role, you will be responsible for delivering high-quality, compassionate care under the supervision of a registered nurse or physician. Provide skilled nursing care to patients in a home health setting, inpatient rehab facilities, or hospice care as directed by a registered nurse or physician. Monitor patient health conditions, administer prescribed medications, wound care, and other personalized interventions. Record and maintain accurate patient medical histories and document care provided. Educate patients and their families on proper home health care strategies and procedures. Coordinate with healthcare team members to assess, plan, implement, and evaluate patient care plans. Conduct regular assessments of patient health, and offer proactive suggestions for improved health management. Proficient in providing patient-focused care Strong communication skills for interacting with patients, families, and healthcare teams Ability to adapt to changing environments and patient needs Detail-oriented with strong documentation and record-keeping abilities Medical Insurance Dental Insurance Vision Insurance 401k Current LPN or LVN license in the state of practice CPR certification
